Documentation goes stale in specific, findable places, and the people who find them are your readers. Most of them will not open a pull request, and most will not open a GitHub issue either — but nearly all of them will type two sentences into a form that is already on the page.
The page URL is the whole report
For docs feedback, the single most valuable field is the one nobody has to fill in. Every report carries the exact page it was written from, so a vague "this is wrong" is still actionable.
File straight into the docs repo
Point the GitHub integration at your documentation repository with a "docs" label. Reports arrive where the content lives, next to the file that needs changing.
Ask one extra question
A single select — "Out of date / Confusing / Broken example / Missing" — turns a pile of reports into a sorted queue at zero cost to the reader.
